Closing the Gap on Racial and Ethnic Health Care Disparities: Lessons from the 2005-2008 Grantees In 2005, the Blue Cross Blue Shield of Massachusetts Foundation launched the Closing the Gap on Racial and Ethnic Health Care Disparities grant program area. Two hospitals and seven community-based providers were funded for three years to design and implement strategies aimed at reducing racial and ethnic health care disparities among people of color living with diabetes, asthma, HIV/AIDS, cardiovascular disease, trauma/mental health, and low birth weight/infant mortality.

Improving Quality and Achieving Equity: A Guide for Hospital Leaders The Disparities Solutions Center at Massachusetts General Hospital, with support from the Robert Wood Johnson Foundation created this guide for hospital leaders - including CEO’s, VP’s, and others that focus on quality, safety, finance, and risk management.

Healthy Massachusetts Disease Management and Wellness: Focus on Diabetes Chronic conditions, such as heart failure, diabetes and asthma are pervasive among Massachusetts residents. These chronic conditions impact residents’ quality of life and contribute to disability and premature mortality. Chronic illnesses are especially prevalent among racial and ethnic groups where gaps in diagnosis and care occur.

Evaluation Reports

CT Health Foundation Oral Health Evaluation Report
This report’s findings demonstrate the Connecticut Health Foundation’s investments in community-based collaboration, research practices/policy and advocacy have clearly had an impact on access to dental care for children enrolled in the state’s Medicaid plan. These findings provide the Foundation with information and recommendations to continue to leverage its strategic investments in oral health.

Leadership Reports

Cross-Sector Performance Accountability: Making Aligned Contributions to Improve Community Well-Being, A Preliminary Paper

The paper demonstrates the effectiveness of the unique approach behind the aligned contributions framework and argues for the inclusion of its components when designing future community leadership initiatives.  In addition, this research provides recommendations and guidelines for public administrators around performance management that will influence effective, equitable, and sustainable cross-sector contributions to community improvement.

A Plan for Action: Key Perspectives from the Racial/Ethnic Disparities Strategy Forum

In 2006, the Disparities Solutions Center convened a Strategy Forum composed of twenty experts from the fields of racial/ethnic disparities in health care, quality improvement, implementation research, and organizational excellence, with the goal of deciding on innovative action items and adoption strategies to address disparities. The forum used the Results Based Facilitation model, and the key recommendations that emerged can be found in this report.
